Palenie "gumy" przez lokomotywę.
Tak wyglądają szyny, gdy jedna z lokomotyw w pociągu z wieloma lokomotywami nie odebrała sygnału STOP:
Wydarzenie miało miejsce w Northern QLD, Australia, ale zdarza się to na całym świecie, gdzie są pociągi zestawiane z wieloma lokomotywami. Jeden z maszynistów w USA widział co najmniej kilkanaście takich przypadków.

this email a few months ago. I was around the house at the time, and his
Dad came and showed us the series of images...
The (extended) back story is that the train was a 40(?) car set carrying
coal in Far North Queensland, with a number of diesel units at the front
of the train, and some in the middle, which were radio controlled by those
at the front. The train had to stop, and the front units braked
accordingly, but the centre loco's never got the command. Hence the
grinding... To recover the train, they simply split the train in half, and
the front half continued on, but I'm not sure of the fate of the rear
half."

"As a Locomotive Engineer with the largest Class 1 Railroad in the United
States, I have seen this more than a dozen times. This is usually operator
error. Either the train had a helper engine on it and the Engineer fell
asleep or the DPU lost comm and the Engineer didn't realize the issue soon
enough. Time to repair? 2 hours tops. MOW employees cut out the rail with
a gas powered chop saw and weld in new rail with thermite."

Wg przepisów nawet na lokomotywach które nie pracują a jadą w składzie pociągu powinien znajdować się konwojent. Moim zdaniem przepis ten wymaga
