The cage wasn't big enough for the shark to get inside. So what i think happened is, the cage broke when the shark slammed into it after getting the bait. Breaking one of the steel bars

When the shark tried to get out the broken bar stabbed him on the side, that is from where most of the blood seems to be coming from.
